
The Cardiometabolic Prevention Clinic at St. Michael’s Hospital sees two groups of patients – those living with, and at risk for, heart and vascular disease. It provides complimentary preventive care in coordination with primary care physicians and specialists.
This unique clinic at the University of Toronto, spearheaded by Dr. Beth Abramson, aims to share best practices in academic preventive cardiology. Her team of cardiac prevention specialists have expertise in modern “precision” medicine to help diagnose and treat cardiovascular risk.
The types of patients we see in our clinic:
- Patients “at risk” for cardiovascular disease: High cholesterol, obesity, diabetes, high blood pressure, and renal impairment
- Patients with established cardiovascular disease: Coronary artery disease, those who have undergone cardiovascular surgery, patients diagnosed with Peripheral Arterial Disease (PAD)

What to expect when visiting our clinic:
1. Initial Visit
In-person
- Blood work is taken in advance of this first appointment.
- Measurement of carotid intima-media thickness (CIMT) test. This test can fine-tune risk assessment for people at risk for cardiovascular disease and is not necessary for those with established cardiovascular disease. (CIMT checklist to see if CIMT is right for you – coming soon).
- CMIT testing can be arranged directly with our clinic. Please note: you do not need to have an appointment with the clinic to arrange a CIMT test. Contact us at 416-864-5627.
- Patients’ risk factors are assessed, and a personalized plan is addressed with patient education material provided.
2. Follow-up Appointment
3-6 or 12 months as needed – Virtual or may be in-person
- Blood work is taken in advance of this appointment. If improvements have been noted, the appointment will be virtual. If not, the appointment will be in person.
- Patients’ risk factors and personalized plan is reviewed and adapted (if needed)
3. Final Follow-up Appointment
1 year – Virtual
- Blood work is taken in advance of this appointment.
- Patient graduates back to primary care provider and/or specialist
How to Register
Family physicians and other specialists may refer patients to the clinic by completing the referral form. Please see referral form for details.
CIMT testing can be arranged directly with our clinic. You do not need to make an appointment with the cardiometabolic team to arrange a CIMT test (CIMT checklist coming soon).
